Enrolment Services Student Forms

The following forms are available in PDF format. If you do not have Acrobat Reader (the plugin to view PDF files) you can download a free version here. Forms that have a fee associated with the service can be submitted through the Fee Payment Portal.

Registrar & Enrolment Services offers a dropbox on campus for students to submit their forms. The dropbox is located around the right-side corner of the Registrar's Office in the T building, where access is available during campus hours. As in-person transactions are currently suspended, rush orders are not accepted in the drop-box. If you have any questions regarding the dropbox, please contact us at geninfo [at] langara.ca (geninfo[at]langara[dot]ca).
